The packet spine can read clean and still be wrong. A team can reuse last month’s policy assumptions even after the workflow changes. The change is often small on paper, like a new connector, a new data source, or a new mission customer. The packet still looks complete because every section has something filled in. The governance route underneath it can be outdated or out of scope.
