A supervisor asks for the AI paperwork before approving a pilot, and eight separate artifacts land on the desk. Each one is familiar on its own, but the set does not read like a decision. The mistake is treating CP-01 through CP-08 like completed checklists that only need to be attached, because a reviewer needs an ordered story and a clear ownership map. A pile can still hide the one missing gate that stops unsafe data transfer. It can also hide the open questions that will come due the first time the workflow changes.
The artifacts also blur together when they repeat the same fact in different words. A reviewer then has to guess which copy is current, and the guess becomes the decision.
