Review staffing for the Scrum Master role on your teams. Are your Scrum Masters effective at helping your teams execute Scrum practices, including the high-discipline practices associated with Scrum’s failure modes?

Insist that your teams use by-the-book Scrum—unless they can show you a quantitative, measured basis for doing something different. (The “More Effective Agile Process Improvement” chapter goes into more detail about measuring Agile process changes.)
